2022 Contribution Statement

Any parishioner who would like a statement of their 2022 Church Contributions for income tax purposes should fill out the information below and either drop the request in the collection basket at Mass or mail/bring it to the Parish Office.

• Church Envelope # (Required)

Contribution Statements will not be automatically mailed. Requests must be made in writing with all the above information provided. Please do not call the Parish Office as requests cannot be taken over the phone. Thank you.
